CASE MANAGEMENT
Case Management provides for the initiation of services needed for a client and the ongoing monitoring of all the services provided. Regardless of residence or location, every client receives assistance through a specialized Plan of Care developed and maintained by our Case Manager, in which five areas of human development are addressed each day: Cognitive, Sensory, Motor, Communicative and Social Behavior.
Our Case Manager is responsible for locating, coordinating, and supervising all needed medical, social, education, and other services, as well as any informal community supports clients may require.
Each Plan of Care is developed with an aim of helping clients achieve maximum independence.
